Extending the impact of County Health Rankings & Roadmaps through a rapid-response grant program

The Foundation's County Health Rankings & Roadmaps initiative was designed to help communities translate the County Health Rankings into multisector action that addresses the social, economic, environmental, and behavioral factors that affect health.This funding will support grants under the Rapid Response Fund, managed and administered by the Robert Wood Johnson Foundation (RWJF) Active Living by Design (ALBD) program, as part of the larger effort to expand the reach and impact of the County Health Rankings & Roadmaps (CHR&R). This fund will incentivize communities to catalyze local action, leverage local resources, and actively engage with existing resources within CHR&R to address multiple determinants of health. Project deliverables will include: (1) recommendations for the structure, process, and management of the rapid-response program; (2) development and dissemination of a call for proposals; (3) development of selection and scoring criteria; (4) establishment of an application-review process; (5) identification of strategies for connecting applicants and grantees to complementary CHR&R resources; and (6) ongoing management and synthesis of the rapid-response grantee communities. In addition, ALBD will collaborate with RWJF and the University of Wisconsin Population Health Institute in all aspects of this effort and serve as an ambassador to the CHR&R program.
